Basically: Discord’s boredom creates a self-contained universe where all of the individual Tumblr blog worlds are born. He made them so that he could travel to a bunch of different worlds and entertain himself. Which means mind-raping and pranking ponies, of course.
BUT THEN Twilight and her friends show up and lock him back in stone. So now the little world are running around on their own accord, telling stories, creating characters, and even dying. The worlds don’t always seem to be aware of each other, but they can all cross over with each other. Some can even link together to form one long continuity.

My Little Pony: Friendship is Magic has a lot of problems aside from the D-issue. Your analysis as to why D. got so much attention is spot-on, but in case of the patient pony, you forget that there is also a timing matter. When ”Read It and Weep” aired, it was only two weeks after the start of the D-drama. Any controversy the patient deserved simply got trampled over by the D-drama. In fact, I’ve seen many bronies (who know nothing about the time it takes to produce an episode) who were convinced that the patient was supposed to represent those angry about D’s portrayal, particularly Yamino, and meant as a jab towards them/her by the FIM crew. So, people understood the patient was an offensive portrayal, but reworked it as a ‘justified insult’ towards those who were displeased with “The Last Roundup”.
That said, while most people understood the patient to be used as an offensive joke, I think it is difficult for most to voice how it is offensive. FIM’s sexist, racist, and (‘regular’) ableist content, sure, most people have at least a basic understanding of how these things work because they’re part of daily life, but how many people will have experience with what goes on in a psyche ward? That lack of knowledge and experience is a factor that both reduces involvement and courage to speak up. For that, people who know what’s what in a psyche ward are needed.
I, for one, am glad you took that task upon you. The patient’s portrayal and treatment bothered me, but I don’t think I’ll ever try writing about the problems surrounding the joke myself because I quite simply wouldn’t know what I would be talking about, which potentially would add to the offense. The most I can do is reblog, so here.
